Dementia support · Medicare GUIDE
After a diagnosis, take one step at a time
A new dementia diagnosis can bring more questions than answers. Reflections helps families think about support at home, including contracted in-home respite care through Medicare GUIDE when eligibility and the care arrangement are confirmed.
Get the Guidance You Need
Leave room for questions and for the person’s voice
You do not need to solve every future care question immediately. Ask the person what matters most to them, what they want help with, and who they would like involved. A care plan should include their preferences rather than becoming only a list of tasks for others.
Choose one or two immediate priorities for the next conversation. Keep a short written list so important questions are not lost between appointments or family discussions.
Organize the first conversations
Clarify the diagnosis with the clinician
Ask what the diagnosis means, who will provide follow-up, what treatment discussions are appropriate, and which changes should prompt a call.
Talk about everyday safety and support
Bring concerns about medication routines, driving, meals, or living alone to the care team. Ask for an individualized assessment rather than making assumptions from a website.
Share the practical responsibilities
Write down who is currently helping, what each person can realistically do, and where there are gaps. Include the caregiver’s own commitments and need for rest.
Begin planning while the person can participate
Discuss preferences for future care and seek appropriate professional guidance about legal, financial, and health-care planning. Keep the person involved as much as possible.
Keep useful information together
A simple folder or agreed place for appointment notes can help the family stay organized. Include the names of current providers, a medication list for the clinical team, and questions for the next visit. Share personal information only with people and services the person has agreed should receive it.
This is about making a conversation easier. You do not need to send that folder through Reflections’ website to ask for help.
Plan for the caregiver as well as your loved one
Make room in the plan for the family caregiver’s rest and other responsibilities. Reflections provides contracted in-home respite care through GUIDE for eligible families, giving you time away while your loved one receives attentive support.
A new diagnosis does not automatically confirm GUIDE enrollment or a respite benefit. We can discuss the starting points and what needs to happen before visits are arranged.
