The starting points for a review

GUIDE eligibility depends on a provider-confirmed dementia diagnosis, the person’s Medicare coverage, where they live, and their current care arrangements.

  • Dementia confirmed by a clinician on the GUIDE participant’s practitioner roster.
  • Medicare Parts A and B with Medicare as the primary payer; no Medicare Advantage, Special Needs Plan, or PACE enrollment.
  • A private residence, or an approved residential care community partnered with the GUIDE participant. Long-term nursing home residents and people residing in a memory care unit are excluded.
  • No election of the Medicare hospice benefit.
  • No current enrollment with another GUIDE participant.

Find a useful next step

Answer six starting-point questions. This is an informational check, not an application, diagnosis, or eligibility decision. The GUIDE care team and Medicare must verify the details.

This checker sends and saves no answers. It asks for no name, date of birth, Medicare number, or contact details.

If the diagnosis has not been confirmed, choose No or Not sure. A provider can explain the assessment needed.

This means Original Medicare as the primary payer. If the coverage arrangement is unclear, choose Not sure.

Medicare Advantage includes Special Needs Plans. These programs have different coverage arrangements.

A residential care community needs an approved partnership with the GUIDE participant. Memory care units and long-term nursing home residence are excluded.

Ask the current care team if you are unsure about hospice enrollment.

Existing GUIDE support must be discussed with the current participant before considering another provider.

A residential community needs a closer look

An approved residential care community may be eligible for some GUIDE services, but its residents cannot receive GUIDE respite. In-home respite through Reflections is for eligible people living in a private residence.

Questions you can bring to the conversation

If you have the information readily available, note the type of Medicare coverage, the name of the current care provider, and the person’s living arrangement. You can also ask whether a recent assessment supplies the necessary diagnosis documentation.

Keep Medicare numbers and medical records out of a general website message. The participant can explain how to provide information through its appropriate enrollment process.
