Job Description

The Clinical Coordinator will be responsible for managing the day-to-day clinical activities, ensuring compliance with healthcare regulations, and facilitating excellent patient care. The ideal candidate will be a decisive leader with a strong background in nursing or a related healthcare field, showcasing superb time management, record-keeping, and administrative skills.

What a Typical Day Will Look Like:

  • Engaging with patients and their families directly, providing care coordination, advocacy, and educational support.
  • Conducting comprehensive assessments upon patient enrollment to inform the development of personalized, comprehensive care plans that address medical, behavioral, and social needs.
  • Identifying social determinants of health during assessments to ensure a holistic approach to patient well-being and integrating this knowledge into the care plan.
  • Working collaboratively with an interdisciplinary care team to coordinate and integrate a broad spectrum of patient needs and services.
  • Managing patient case education and overseeing the documentation of patient visits and progress to ensure quality care and regulatory compliance.
  • Recruiting, training, and overseeing staff to maintain a high standard of patient care and confidentiality while fostering a supportive and efficient team environment.
  • Designing and overseeing departmental budgets, ensuring resources are used effectively to support patient care and company operations.

Position Goals:

  • To ensure the seamless operation of clinical services by effectively coordinating all clinical activities.

Qualifications:

  • Nursing degree or degree in a health-related field.
  • At least 2 years of experience in a clinical coordinator role within a healthcare setting.
  • Strong ability to problem-solve and implement innovative solutions.
  • Excellent organizational skills and meticulous attention to detail.
  • Capacity to work effectively under pressure and in emergency situations.
  • Competent in working independently and collaboratively within a team setting.
  • Outstanding documentation, communication, and IT skills.
  • A passion for maintaining clinical excellence and improving patient outcomes.
